Individuals Have the Following Types of Disabilities:
Deafness/Severe Hearing Impairment, Autism, Behavioral Problems, Orthopedic Impairment, Blindness/Severe Visual Impairment, Cerebral Palsy, Chemical Dependency, Epilepsy/Seizure Disorder, Head Injury, Mental Disorder, Mental Retardation, Multiple Sclerosis, Muscular Dystrophy, Specific Learning Disability, Speech/Language Impairment, Spina Bifida, Spinal Cord Injury, Neurological Impairment, and other Undetermined disabilities.

Describe your quality assurance practices, including any agency accreditations

Talbot County Resource Coordination in concert with the other eight counties of the Eastern Shore and the Eastern Shore Regional Office have representatives to the Quality Assurance Committee. Annual surveys are sent to individuals receiving services and the information received his compiled and a "report card" developed. Each Resource Coordination office uses the information to enhance and improve customer service.

How Does Your Agency Support People In Becoming Active Members Of Their Community?

One of the primary roles of resource coordination is to facilitate and monitor the delivery of services. Services are designed based on individual preferences and needs and are community based. Resource Coordinators encourage and support individuals to access the wide array of commmunity resources and encourage their participation in churches, clubs, recreational and social activities.

Resource Coordination is licensed by the Office of Health Care Quality.

Steps Individuals Must Take to Apply for Services

The application for services can be obtained online at www.ddamaryland.org or from the Talbot County Health Department at 410-819-5600. Once an application is received, the person will be contacted to schedule an intake interview to begin the eligibility process. It is imperative that supporting documentation of the type and onset of the disability be provided. Examples of acceptable documentation are psychological and psychiatric evaluations, school records, and evaluations from your doctor. A needs assessment will be completed, during the intake process. The information is reviewed, justified, and sent to the Eastern Shore Regional Office for eligibility determination.
