How Does Your Agency Support People In Becoming Active Members Of Their Community?

Innovative Services locate individuals in the community of their choices, and organize regular outings within the community. Innovative Services receive information from the Service Coordinators about interesting activities in different communities. Innovative Services involves individuals in preferential activities such as bowling, movie, swimming, church, mall/shopping, sporting event, yard sales, and amusement park.

Steps Individuals Must take To Apply For Services:

Most of Innovative Services' referrals come from the Service Coordination offices. However, when individuals do contact Innovative Services directly, it usually refer them to service coordnation and assist them by providing information and options. Innovative Services then schedules a meeting with the individual and family, along with the service coordinator. This meeting gives Innovative Services the opportunity to explore potential compatibility, and answer any questions about its services. Innovative Services also uses the meeting as an opportunity to understand the needs of the individuals, and whether or how it can meet these needs. If there is a mutually beneficial interest Innovative Services then works with the service coordinator to ensure that there is DDA funding and eligibility. The final step is to begin developing the service funding plan and submit it to DDA (CST) for approval.
