Describe your quality assurance practices, including any agency accreditations

The region has developed a Quality Assurance committee that meets regularly to complete and review an annual survey. They then take the response on the survey to create a "report card" for each county to review and use as necessary for improvement.

How Does Your Agency Support People In Becoming Active Members Of Their Community?

The Health Department encourages and empowers people to become participants in their own services and not simply observers.

Steps Individuals Must Take to Apply for Services

An individual (or other interested party), may either walk in to the local Health Department OR call in advance to schedule an appointment with a coordinator to discuss service requests.

To apply for Developmental Disabilities Administration (DDA) funded services, an individual will need to complete a DDA Application for Services form, and a Critical Needs Recommendation form. They will also need to submit with their application some documentation of their disability. These forms may be obtained on-line or in person from any Resource Coordinator.
