The Washington County Human Development Council, Inc. offers Residential, Day Habilitation, Supported Employment, and Medical Day Services to adults that are medically handicapped, physically challenged, and developmentally disabled. The Washington County Human Development Council Inc offers services in Washington and Frederick counties.

The focus of Washington County Human Development Council, Inc is to promote independence and raise the quality of life for the individuals that they serve.

Discuss How Your Agency Has Utilized The "Ask Me!" Survey Results.

The agency's quality assurance committee reviews the "Ask Me!" survey, making recommendations to the departments to continually improve the agency's programs. Areas of focus are developed and formed into agency goals.

The Washington County Human Development Council, Inc maintains a quality assurance committee known as "The Standing Committee". This committee is composed of parents, program participants, staff, and concerned citizens. The committee meets monthly to review behavior plans, rights issues, and incident reports. Annual goals are set by the committee which require members to personally review the homes and centers, review individual consumer satisfaction surveys, and recommend agency policies changes.

How Does Your Agency Support People In Becoming Active Members Of Their Community?

Day and residential programs encourage and support community integration. The Washington County Human Development Council Inc assists and encourages individuals to participate in local churches, clubs, and community associations.

Steps Individuals Must Take to Apply for Services

The Washington County Human Development Council, Inc would be happy to arrange a tour of its facilities for DDA eligible individuals and their families. If Washington County Human Development Council, Inc is selected to provide services Washington County Human Development Council, Inc will complete a Service Funding Plan which is reviewed by the Service Coordinator and the family. Once approved by DDA Washington County Human Development Council, Inc will contact the family or guardian with a start date and transportation arrangements. An individual Planning meeting will be held shortly after admission.

Persons that are applying for Medical Day Care Services may download the 3871B and the "Additional Information Form" from http://www.wchdc.org. Have your personal physician complete the forms and Washington County Human Development Council, Inc will submit them to the State for approval. Facility tours are also available for interested persons.
