Mission Statement

CPL's mission is to encourage and support each person in making choices and in gaining skills that will enae the individual to participate in the diversity and richness of community life. CPL strives to assure that each individual enjoys the best quality of life by advocating for access to community based services and activities.

Unique or Distinctive Qualities

CPL initiated the pilot project for the individual family care program in 1988. CPL is currently the largest provider of Individual Family Care (IFC) services in Maryland.

Discuss How Your Agency Has Utilized The "Ask Me!" Survey Results.

The "Ask Me" survey results have given CPL the opportunity to improve the delivery of services and to enhance the opportunities available to the individuals in its IFC program to excercise their rights and to engage in a wider range of community activities. As a result of the "Ask Me" survey, CPL initiated efforts to assure that its clients are registered to vote.

Discuss your quality assurance practices, including any agency accreditations

CPL's community placement coordinators visit each IFC home at least monthly to observe the environment and to discuss the individual's activities, needs and concerns. They also meet with each individual bi-monthly to talk about what has been happening in their lives and what, if any, changes they would like to make.

CPL's human rights committee meets every three months to review incidents affecting the rights, health, and well being of individuals. The committee also reviews and approves all behavior support plans. When necessary, the committee makes recommendations to the management team addressing areas that require revision or improvement.

The management team performs a quality assurance review annually to determine how well the agency has complied with its internal quality assurance plan. CPL conducts satisfaction surveys annually with the consumers and the care providers

How Does Your Agency Support People In Becoming Active Members Of Their Community?

The IFC care providers act as surrogate families for its consumers and strive to include each individual in family and community activities. Its individual family care program offers each individual a wide array of opportunities to interact with neighbors, to form friendships, and to participate in social/recreational activities.

CPL's community placement coordinators and nurses work together with the individuals and the care providers to coordinate services such as healthcare, counselling, and job placement. In addition, CPL has a partnership with the Best Buddies progam at Villa Julie College to match students with consumers for community social and recreational activities. A number of its consumers are also active in Special Olympics.

CPL also supports the group, "People on the Go" in their advocacy efforts. Additionally, CPL has worked actively to promote voter registration among its consumers.

Steps Individuals Must Take to Apply for Services

Special Considerations/Comments: Since CPL provides services in private homes, they are unable to accept individuals who require awake overnight care or intensive behavioral supports

Funding Sources: Developmental Disabilities Administration, Department of Human Resources, Social Security benefits and Private pay.
