Mission Statement:

CIS&H, Inc. is a private child care residential program incorporated in the State of Maryland to provide habilitation and case management services to male children with severe, chronic developmental disabilities, and emotional disturbances.

Unique or Distinctive Qualities:

CIS&H offers the widest choices for children with developmental disabilities. Its philosophy is based on the belief that every individual has the right to fully develop to his or her potential. Basic to its system of service is the conviction that active involvement of primary and secondary consumers is critical to success. Equally critical is a well-trained dedicated staff and responsive Board of Directors who aspire to excellence in providing the range of its services. Fundamental to this expansion is the maintenance of quality. Individualized supports and services are implemented in a manner which is culturally normative, age appropriate, and focus on individual strengths, needs, abilities, interests and desires. To facilitate implementation, services are provided to each individual in the least restrictive environment appropriate to his or her needs, by trained, caring staffs who provide support in all areas of everyday living.

Discuss How Your Agency Has Utilized The "Ask Me!" Survey Results.

CIS&H's experience with "Ask Me!" is limited. Although it realizes the value that could be added by full implementation, CIS&H is in the process of restructing its program operations and communications in order to better serve its clients and service providers.

Discuss your quality assurance practices, including any agency accreditations

At CIS&H, Inc. it is realized that quality and its assurance is an on-going process. To achieve this objective, CIS&H rely on self-adults and random site and facility visits. Its Board of Directors are also deeply involved in its quality assurance practices. Members visit facilities randomly and provide invaluable feedback concerning all facets of the operation. CIS&H has not applied for accreditations

How Does Your Agency Support People In Becoming Active Members Of Their Community?

CIS&H's clients are encouraged to become actively involved in the community by regular attendance in local or alternative school-based work projects, community-based social and recreational activities and outings, as well as other critical life-skills growth and development requirements. CIS&H strongly encourages its young men to be good, helpful and respectful to all members of the community as well as their immediate neighbors – its houses are their homes.

Steps Individuals Must Take to Apply for Services:

Individuals may apply for services through on-site visits to ne of CIS&H's facilities, referrals from Resource Coordinator, or by completing CIS&H's company application.

Special Considerations/Comments: CIS&H, Inc. does not currently serve young men who have a history of complex medical needs or fire starters.
