- Contact Resource Coordinator in county of residence
- Complete application for services
- Resource Coordinator develops a Person Centered Plan
- Agency submits Services Funding Plan
- When approved for funding services commence
Special Considerations/Comments: ISS/FSS/CSLA programs provide minimal supports so referrals must self administer medications, live safely without supervision, and be stable (mentally and physically)
