Steps Individuals Must Take to Apply for Services:

For Day, vocational, and support services, individuals complete CHANGE's application forms as well as Developmental Disabilities Administration's forms. A personal interview with its Program Director and a visit to its day and vocational program are also part of the process

For Respite Inn, the individual and family meet with the Respite Inn Manager, and have a nursing assessment by CHANGE's nurse prior to utilizing the Inn.

Special Considerations/Comments: CHANGE receives numerous requests for Day, Vocational and support services from persons residing in neighboring counties. This is due to its reputation for providing services that are designed to meet individual needs.
