Prior to regulations requiring agencies to develop a Quality Assurance Plan, Bayside Community Network already had internal review practices in place. The Quality Assurance division of Bayside Community Network reviews the agency from an internal perspective; looking at each component as well as the agency as a whole. Agency consumers make up the majority of the Quality Assurance team and are accompanied by family members, direct care staff, community members and administrative personnel. Although the focus may change with the evolving needs of the agency, primary focus is given to the medical health of consumers, behavioral supports, staff education and employment potential for all consumers.

How Does Your Agency Support People In Becoming Active Members Of Their Community?

Bayside Community Network has spent 34 years developing community connections for the consumers its serves, Its consumers are active members of civic organizations, churches and community organizations. Through a partnership with the local community college, Bayside currently has 20 individuals receiving continuing education courses. Bayside has a very high success rate in assisting individuals obtain indpendent employment in the community. Many long lasting relationships have developed as a result of individuals successful employment.

Steps Individuals Must Take to Apply for Services

Individuals interested in services must first contact DDA Resource Coordination at (410)996-5140. Resource Coordination will assist the individual to set up a date and time to tour the facility. Once the individual determines that he or she would like to attend the program the Resource Coordinator will write a Person Center Plan detailing the specific needs of the individual. Once the Person Center Plan is received by Bayside, the admissions committee will determine if the agency has adequate space and the skills needed to meet the needs of the individual. If the committee agrees then the agency will submit a Service Funding Plan to DDA to ensure the dollars to provide the services. Once an award letter has been issued then services can begin.
